Passport applications are accepted Monday
through Friday between 8:00 a.m. and 4:30 p.m.
Pricing will be as follows:

You will need 2 passport photos, certified
copy of your birth record, and a photo ID (hospital certificates are not
accepted). Applications must be signed before acceptance agent.
There will be a 5-7 week processing time. For
quicker processing, a $60 expedite fee will be collected at the time your
application is submitted. Overnight prepaid envelopes should be provided
also.

Passport Fees
Applications for passports must be brought to the Recorders Office for
processing. Passport applications may be obtained at our office or the
National
Passport Agency.
ITEMS NEEDED TO OBTAIN A PASSPORT:
1) Completed Passport Application
2) Two passport pictures
3) CERTIFIED COPY of Birth Certificate (Birth certificate from the hospital DOES
NOT meet standards) or ORIGINAL Citizenship Certificate
4) For applicants 18 years of age or older:
a) Drivers License or other PHOTO ID
b) Fee: $110.00 – must be check or money order payable to US Department of
State
c) Fee: $25.00 – may be cash or check payable to Carroll County Recorder
d) Fee: $30.00 - for a passport card must be check or
money order payable to US Department of State (Optional)
5) For applicants 16 or 17 years of age:
a) Must appear in person with their driver's license and a check signed by
a parent. If applicant does not have a driver's license, one of the parents
must appear with a permanent driver's license.
b) Fee: $110.00 – must be check or money order payable to US Department of
State
c) Fee: $25.00 – may be cash or check payable to Carroll County Recorder
6) For applicants age 15 or under:
a) BOTH parents or legal guardian MUST sign the application, child DOES NOT
have to appear in person. Ask for details if one of the parents is unable to
sign the application.
b) Fee: $80.00 – must be parent's check payable to US Department of State
c) Fee: $25.00 – may be cash or check payable to Carroll County Recorder.
ALLOW 6 WEEKS FOR PROCESS OF PASSPORT, YOU MAY EXPEDITE FOR "$60.00 AND A
2-WAY OVERNIGHT DELIVERY ENVELOPE."
DO NOT SIGN APPLICATION UNTIL YOU ARE IN THE PRESENCE OF A PASSPORT
ACCEPTANCE AGENT.
